Pathway plans

Your pathway plan is about your needs and what we all need to do to ensure your successful transition to adult life. You will continue to have a pathway plan until you're 21 (or longer if you're in education or training or if you decide you want support until you are 25).

Personal Adviser

If you are eligible for leaving care support, you will be allocated a social work assistant at seventeen and a half years old, who will act as your named ‘personal

advisor’ and help you to prepare for living independently as you approach your 18th birthday. You will have a Pathway Plan which outlines your plans and needs and this will be reviewed with you every six months.

 

Your Personal Adviser will continue to support you up to the age of 21 and up to the age of 25, if this is what you want.
